| Withdrawal / Scheme Certificate |

| If you are a member of Employees Pension Scheme 1995 and resigned
from BPCL with less than 10 years of service and wish to opt for withdrawal
of EPS contribution or Scheme Certificate, you can fill in Form 10C
and send it to the Regional HRS set up of BPCL. They will attest your
form and arrange to submit it to the Regional Provident Fund Commissioner. |

Transfer to New Employer |
| |
| If you have resigned from the service of BPCL
and joined another employer, please fill in Form 13 to transfer your
PF/EPS balance to the new employer. |

| If you are a member of Employees Pension Scheme 1995 and retired
from the Company’s service on attaining the age of superannuation,
